NMDP Pathways to Transplant: Support for you and your patients

Your patients’ path to hematopoietic cell transplant (HCT) or cell therapy starts with you. NMDPSM Pathways to Transplant helps you navigate decisions, coordinate care and access the right resources for your patient from diagnosis through long-term follow-up.

Supporting practices. Reaching patients.

Pathways to Transplant connects hematology/oncology practices and patients to the broader NMDP Network—supporting earlier action and ongoing care from diagnosis through survivorship.

What it’s like to partner with the Pathways team

Two people shaking hands

You'll have access to clinical experts who understand the realities of hematology/oncology and transplant care—so you can move patients forward with confidence.

All our Pathways to Transplant team members have worked in the hematology/oncology or transplant setting.

  • You'll connect first with a medical science liaison who’s a nurse practitioner or physician assistant.
  • We'll work alongside your practice—and the transplant center—to weave programs and services into your workflow.
  • You'll have ongoing, as-needed support from a provider relations manager.

Support your patients and their loved ones with resources you can share

The NMDP Patient Support Center makes it easier to support your patients beyond the clinic visit. Guide your patients to one-on-one, tailored support and share clear, expert-reviewed resources that help them understand transplant, prepare for what’s ahead and navigate recovery.

Most resources are available in English and Spanish with many in up to 12 languages. Access key resources below or explore our full library.

You don’t have to be an expert in transplant to use Pathways. You just have to have the discussion [with the patient] that transplant is an important tool and have the HLA Today kits.
