Learning Objectives

• Document and find previously documented donor information in STAR Link regarding preferred name, gender, and pronouns
• Assess and document which questions and applicable sub-questions of the HHSQ need to be completed
• Communicate and answer PE provider questions on whether GU exams need to be performed, and how to complete the Addendum to the Physical Exam (F00806) question on MSM contact
• Complete Forms 700 and 705, scheduling pregnancy tests and assessments as needed
• Assess and document relevant eligibility questions relating to sexual contact
